This document is not a form to submit to the court; instead, it serves as a guide for you, the victim of a crime, to help you gather the necessary documentation and information for making a restitution request.

This is not the same as the Ohio Crime Victim Compensation Program (found here), which is a program that may be able to reimburse you for expenses incurred as a result of the crime, regardless of whether the offender has been located or arrested.
